Application and Appeal Forms

A single CR3 form now exists for domestic and non-domestic properties, which should be used to apply to have a new property valued, to have alterations assessed or to query your valuation.

• Valuation Application Form (PDF 268 KB)

This is a writeable document, which means that you can complete on screen, print and send to LPS. Alternatively, you can save the document to your desktop, complete the form and send as an attachment to: you local LPS Valuation office. You can find details of your nearest office by following the link at the bottom of the page.

• Valuation Appeal Form (PDF 125 KB)

This form is to be used if you have received a District Valuer's Certificate revising your valuation and you wish to appeal that decision to the Commissioner of Valuation. (There is a strict limit of 28 days from the date of the Certificate within which you may make an appeal application. If you are outside the 28 day limit then use the Personal Application Form above instead and returned this to the Central Registration Team at the address on the form.) The Completed Appeal Application Form should be sent to the Commissioner of Valuation, Land & Property Services, Queen's Court, 56-66 Upper Queen Street, Belfast, BT1 6FD.
